Claims Process

Any communications regarding your claims will be from Dykema (Receiver's counsel) or Stretto (Receiver's claims agent).  The Receiver and her team (Dykema and Stretto) will NOT request any banking information from you.  Please DO NOT provide any information to anyone who solicits or otherwise claims to represent the Receiver or provide assistance to claimants. 

All allowed claimants must provide the Receiver with a completed and signed IRS Form W-9, which is available at https://www.irs.gov/pub/irs-pdf/fw9.pdf .  Please call 210-554-5845 to request a mailed IRS Form W-9.  

The completed and signed IRS Form W-9 form can be mailed to Dykema Gossett PLLC c/o Heartland Receivership, 112 East Pecan Street, Suite 1800, San Antonio, Texas 78205 or emailed to heartlandreceivership@dykema.com.  The Receiver will only issue distribution checks directly to the allowed claimant.
